The Fountain of Life Outreach Ministries Inc. membership is open to any person, Non-Profit Organization, ministry, school or Para church group that is in agreement with the Fountain of Life Outreach Ministries Inc.

Every applicant must fully agree with the Doctorial or Missions Statement.

Every applicant must realize that he/she is a potential representative of Fountain of Life Outreach Ministries Inc. and must be willing to uphold a standard of respectful behavior and our Code of Ethics.
 

Every applicant must honor the  and a One-Time Membership Set up Fee $325.00 along with a monthly membership Fee $50.00. Our office must receive this fee before any application will be considered. These funds are payable in US funds only, by money order, personal checks, certified check only will be accepted. Credit card payment is acceptable if the proper information is supplied through PayPal.

Every applicant must honor a Annual Renewal $200.00 and a Annual Financial Report to continue in good standings with FOLOMI. Failure to do so will result in his/her being suspended from membership and Tax Exemption Status with the IRS.

Every applicant should know that dismissal from Fountain of Life Outreach Ministries Inc. would also result in the suspension of all  involvement with, Fountain of Life Outreach Ministries and any 501(c)(3) Tax Exemption Status with the Internal Revenue Service
